Meaningful Opportunities for Successful Transitions
A unique, one-year program of services and support for young adults with disabilities who need, but have not found, a placement in an appropriate day program after aging out of educational entitlements. Helps fill a pressing need in the community by providing services to individuals who need, but have not found, a placement in an appropriate day program after aging out of educational entitlements. Offers an individualized program of activities and training based on each participant’s assessment and goals. Serves as a portal for individuals and their families, introducing them to a broad range of community resources which will, in most cases, include an introduction to and collaboration with outside vocational or day habilitation programs. This collaboration will help individuals identify and become familiar with programs they might wish to enroll in, and allow outside service providers to likewise become more familiar with an individual’s capabilities.
Target Population : JFGH serves men and women over the age of 18 with one or more of the following: mobility impairments, developmental disabilities, chronic mental illness, deafness or visual impairements.

The Mary and Charles Oshinsky Apartment Program
OAP currently serves residents with learning disabilities, mild mental illness. Secondary and tertiary disabilities include cerebral palsy, Down Syndrome, autism, seizure disorder, muscular dystrophy, head injury and dual diagnosis (developmental disability and mental illness). Ages of residents range from the early 20's through late 60's. Some residents have valid driver's licenses. Nearly all others access public transportation independently. Two thirds of the OAP residents are competitively employed; several participate in supported employment or regular volunteer work and several are currently unemployed.
Target Population : JFGH serves men and women over the age of 18, have there own apartment/condo, and have a developmental disabilities or chronic mental illness who can live independently but can benefit from drop-in support.
