Palliative Medicine Consultants, Inc

Palliative Medicine Consultants provides support to patients and families with life limiting diseases. Services provided include symptom management, pain management, and consultative decision making. Social Worker and Chaplain are part of an integrated interdisciplinary team approach. Patients can continue to receive aggressive, and curative treatment, while simultaneously receiving palliative support.

 

Target Population: In order to receive a palliative consultation, the patient must have an life limiting disease. Palliative Medicine does not take over the primary care of the patient, rather, provides an extra layer of support

Intake/Eligibility: To get started, a referring from the primary care physician, or specialist must be received.
