Programs

Montgomery Cares (MCares)
The Primary Care Coalition administers Montgomery Cares and is responsible for coordinating the services of the participating health care providers who share a common mission and work collectively to provide high-quality, efficient, accessible, equitable, and outcome-focused health services to low-income, uninsured adults in Montgomery County. Montgomery Cares is a public-private partnership composed of 10 independent safety-net primary care clinics, six hospitals, the Montgomery County Department of Health and Human Services, and the PCC, as well as volunteer health practitioners and other community-based organizations.
 
Target Population: Serves adult (18+) residents of Montgomery County whose household incomes are less than 250% of the Federal Poverty Level and who are not eligible for any state or federal insurance programs.
Project Access
Project Access is a specialty care referral network administered by the Primary Care Coalition (PCC). It is composed of a dedicated group of health care specialists, local hospitals, and diagnostic facilities that participate in a coordinated effort to provide specialty care to low-income and uninsured members of our community. Project Access helps these individuals gain timely access to specialty care and assists providers with care coordination and administrative support.

Montgomery Cares Behavior Health Program
The Montgomery Cares Behavioral Health Program (MCBHP) is a collaborative effort of the Montgomery County Department of Health and Human Services (DHHS) and the Primary Care Coalition (PCC) of Montgomery County, a private non-profit charitable organization working with public/private partners to provide high-quality, accessible, equitable, efficient, and outcome-driven health care services for low-income, uninsured County residents. The MCBHP provides a care management team (Behavioral Health Care Manager, Psychiatrist, Psychiatric Services Coordinator, and Community Resource Coordinator) to work intensively with patients with behavioral health needs.
 
Target Population: Serves adult (18+) residents of Montgomery County whose household incomes are less than 250% of the Federal Poverty Level and who are not eligible for any state or federal insurance programs.
Medicine Access Community Pharmacy/MedBank
Provides generic medication, diabetic supplies, immunizations, and FIT kit (colorectal health screenings) to MCares patients free of charge (brand name drugs must be acquired from the manufacturer).

Breast and Cervical Cancer Program
The goal of BCCP is to strengthen the population health using evidence based clinical interventions to increase the breast and cervical cancer screening rates among safety net clinic patients in Montgomery County. PCC plans and implements training and technical assistance activities to assist primary care clinics with assessing and enhancing their capability to increase breast and cervical screening rates through quality improvement processes and the implementation of EBIs.
