Programs
Residential Rehabilitation Service
The Rock Creek Foundation’s Residential Rehabilitation Program
promotes an individual’s ability to engage and participate in
appropriate community activities of their choice, in a supportive
environment, while enabling the individual to develop the daily living
skills needed for independent functioning. We provide goal-directed
psychiatric rehabilitation services that facilitate rehabilitation and
integration into community life.
• Adults with severe and persistent mental illness
• Adults with developmental disabilities and mental illness
• Adults with emotional and/or behavioral challenges
The services are pre-authorized by the LBHA or DDA of the jurisdiction
in which you reside
Day Habilitation/ Meaningful Day Services
Day Habilitation Services assist individuals in acquiring, retaining,
or improving the self-help skills related to activities of daily
living and the social and adaptive skills, which are necessary to
reside successfully in home and community-based settings. RCF
facilitates and supports full participation of individuals with a
disability in the fabric of everyday community life. We use integrated
living, learning, working, and social environments and the utilization
of natural supports and generic resources to achieve our goals which
reflect our values. Additionally, the program provides behavioral,
social, recreational, and practical life skills development which may
also be offered in community based real life settings including the
client’s home, place of work, or caregiver’s home.
Adults with developmental disabilities and severe and persistent mental illness
The services are pre-authorized by DDA. Additional referrals can come
from Community Coordination Services, schools, fairs, and other
community bases resources.
Community Development Services
Community Learning Services are provided in integrated environments
where people have opportunities to connect with and participate with
other community members. Persons interested may include those who are
retired, employed in the community less than 4 hours per day, or
persons who would like to explore varying or alternate employment
goals. Community Learning Services differs from Day, Vocational, or
Support Services in that a majority of the thirty hour service week
will take place in integrated community activities and will focus on
areas of job development, self-improvement, and health/wellbeing.
Specific supports will assist people in developing the skills and
social supports necessary to gain employment, developing community
connections, promoting positive growth, special assistance, and
education. Services will be individualized and based on the individual
plan of the person served.

Employment Services
The Rock Creek Foundation’s Supported Employment Program provides
community-based competitive employment opportunities matching the
values, skills, and interests of our consumers. Our goal is to help
facilitate employment opportunities that deliver real jobs in real
industries for adults with severe and persistent mental illnesses and
developmental disabilities. The sequence and duration of specific
services are determined by individuals based on their specific needs.

Psychiatric Recovery Programs
Our psychiatric recovery services provide a multi-faceted range of
services for individuals with chronic mental illness with or without
the complicating problem of alcohol/substance abuse. Our focus is to
assist individuals in recovery in reaching their desired goals. We
believe in person-centered care and the use of a strengths based
approach. We utilize evidence-based practice models of care. An
emphasis is on developmental and daily living skills that support each
person’s unique ability to live in the community, to recognize
symptoms of potential relapse, and to use strategies to prevent it and
enhance personal independence. Based on need, consumers may attend on
a part-time or full-time basis.

Mental Health Vocational Program
As part of the supported employment service, our Employment
Consultants help consumers choose a job that matches their abilities,
their interests, and their values. This process is aided through pen
and paper assessments, observational interviews, and work trials. When
the consumer has identified a job that matches the above criteria, a
job search begins that may include pre-employment training that helps
the person complete a resume, prepare for interviews, and gain basic
skills in salary negotiations. Once a consumer obtains a job, the
Employment Consultant will provide intensive support to the consumer
by assisting in the job training, acclamation and developing natural
supports on the job.
