Programs

Residential Rehabilitation Program
provides comprehensive and individualized psychiatric rehabilitation services at all levels of care for individuals who choose to live in a Cornerstone Montgomery operated residence. Cornerstone Montgomery operates 71 properties utilized for residential services, with a total capacity of 224. Has specialized teams who work with substance use and personality disorders.
 
Target Population: Ages 18 and up
Intake/Eligibility: Referral forms and process are listed on our website. Programs vary. Most can be a direct referral, family, hospitals, other agencies and providers. Residential referrals are managed by the County. Once a referral form is submitted, then an intake appointment will be scheduled. Once that is complete then appropriate services will be identified and begun.
Supported Living Program
provides the same comprehensive and individualized supports as the Residential Rehabilitation Program for individuals living in their own home, or living in housing provided by other nonprofit organizations in the community. Includes co-occurring substance use services. This program serves nearly 300 individuals throughout all regions of Montgomery County.

Assertive Community Treatment
ACT is an evidence-based practice that improves outcomes for people with severe mental illness who are most vulnerable to homelessness and hospitalization. ACT utilizes a multi-disciplinary team approach, providing services directly in the community, maintaining a small staff to consumer ratio, and works with consumers for as long as they need. ACT works with consumers on medication management, physical health needs, housing, substance use, and employment goals.

Supported Employment Program
Back to Work Program is an Evidence Based Supported Employment program that helps people recovering from mental illness choose, find and keep employment. This program serves almost 500 individuals per year, and has an employment rate of nearly 68% compared to the national average of 19%. Career Transition Program serves individuals ages 16 and above and who are in the process of completing high-school. Career Academic and Psych-Rehab Services program serves youth ages 18—25 who have completed high-school and are transitioning into the adult mental health/social service system. Also addresses substance use needs.

Outpatient Mental Health Center
Cornerstone Montgomery’s OMHC provides a full range of mental health, substance use, psychiatric and therapeutic treatment services. OMHC serves clients of the Cornerstone Montgomery Psychiatric Rehabilitation Program, as well as members of the community. Cornerstone Montgomery operates three outpatient mental health clinics in Bethesda, Silver Spring, and Rockville, with a service capacity of over 1,650 individuals annually.

Vocational Program
The Vocational Program within Cornerstone Montgomery helps people recovering from mental health disorders choose, find, and keep a job. Cornerstone Montgomery’s strong belief in individual interests, abilities, and self determination is supported by a program of pre-placement counseling, resume preparation, placement assistance and on the job coaching and support. The Vocational Program includes, supported employment, job search support, job keep support, job development/placement services, and follow along support as long as necessary. Clients of the Vocational Program have an employment rate three to four times the national average. Cornerstone Montgomery participates in the national movement of evidenced based practices. Supported Employment is integrated with treatment. Competitive employment is the goal. A job search starts soon after a client expresses interest in working and follow along supports are continuous. Employment Specialists work with candidates to successfully match each person to the appropriate job and continue to support the employer/employee relationship as long as necessary. The supported employment staff works closely with all other providers, the clients’ natural support system and employers to coordinate services and successful outcomes for the client. There is focus on promoting independence, continuity of care, skill development, job acquisition and retention. Staff also provide technical assistance and on-the-job support to the employer in such areas as compliance with ADA regulations and/or how to access tax credits and deductions

Montgomery County Case Management/Housing Facilitation
Contracted case management for individuals not otherwise eligible for the public mental health system. Services: The program links community based services and resources, crisis intervention services and facilitating consumer relationships with individuals and organizations in the community which the consumers find supports. Case management is time limited and is typically available to consumers for a period of up to nine months. The housing facilitation services funded through this program are intended to assist consumers who are tenants of Housing Unlimited Inc. and Hughes Neighborhood Housing with maintaining their independence.

Psychiatric Rehabilitation Program
Provides psychiatric rehabilitation and support services to clients who live in the community, The PRP is an “umbrella” over other Cornerstone Montgomery programs which include Residential, Supported Living and Life Skills. All psychiatric rehabilitation services we provide are individualized and flexible based on the changing needs of each individual. Cornerstone Montgomery has developed an integrated, individualized, and geographically based model for providing psychiatric rehabilitation services. Under this model, staff members from all PRP programs, as well as Outpatient Mental Health Clinic, join together with the common goal of high quality service provision.

Foundations Day Program
Foundations is open to all participants in our Residential and Supported Living Programs, and Vocational clients who have a qualifying diagnosis. An alternative to traditional day programs, groups are psychoeducational and designed to nurture self-esteem, develop community living skills, and improve interpersonal relationships. Foundations fosters recovery through a variety of groups as well as individual training to build confidence and proficiency in all phases of daily life. Classes and activities are offered during the day, evening and on weekends, and include Friday night and weekend social and recreational activities. Some examples for classes include: Recovery, Greenhouse, computer lab, cooking classes, Illness Management and Recovery (IMR), art classes, fitness classes, independent living skills classes and classes focused on social skills.

Residential Crisis Services
Cornerstone Montgomery’s Residential Crisis Services are community based, alternatives to inpatient hospitalization for people in acute psychiatric crisis. 24 hour counseling and crisis stabilization residence located in subur­ban single-family neighborhoods, these programs provide short-term interven­tions on referral. The programs seek to reduce unnecessary admissions to inpatient mental health facilities. Clients are encouraged to participate in structured groups, individual counseling, and crisis stabilization services. It is the intent of the program to enable client’s to go to work, see family and participate in regular therapeutic treatment in order to reduce the disruption to their already disrupted life that their crisis has caused. The length of stay at these programs averages 10 days and serves eligible individuals throughout Montgomery County.The program is based on a zero-reject model that favors more client control and individualized care. Clients have the opportunity to enjoy socialization in a safe, nurturing environment. Individual treatment plans are designed to assist clients in planning crisis stabilization and prevention of subsequent crisis. The program was the subject of a National Institute of Mental Health (NIMH) sponsored research study comparing the treatment and cost effectiveness of the program with that of a psychiatric ward of a local hospital. The study demonstrated that the Residential Crisis Services hospital alternative is equally effective treatment at less than one-third of the cost.
